In order to make the best use of your time in our office, we recommend that you complete our new patient form(s) prior to coming in for your first appointment. Along with these forms, please come prepared with your photo ID and any applicable insurance.

During your initial visit to our office, Dr. Dave will perform a personal consultation and exam in order to determine the source of any pain you may be experiencing.

This will help when prescribing the most appropriate course of treatment based on your condition. Periodically throughout your treatment plan, follow-up exams will be performed in order to map your progress.

During your first visit to our office, we will take X-rays for diagnostic purposes. We are proud to offer advanced digital X-ray technology that allows us to reduce our carbon footprint while caring for our patients.

However, if you have recent x-rays (taken in the last 6 months), we may be able to use the images provided they are of clear quality and cover the areas needed for diagnosing. If you have questions about transferring x-rays to our office.
